Making a Referral
Information for Referrers
Health Opportunities Team can offer one-to-one support to young people between the ages of 12 and 25 who live in the Craigmillar/Portobello and Liberton/Gilmerton areas of Edinburgh. We also offer one-to-one support within some schools in South West of Edinburgh.
Health Opportunities Team One-to-One support aims to help young people to learn strategies to cope with their difficult feelings. Our programmes are most effective when supported by parents/carers or by agencies and workers who are involved in the young person’s life. Our programmes cannot ‘fix’ all the problems faced by a young person but they can help the young person find ways to make positive changes when they are ready to.
To make a referral
If you would like to refer a young person to our one-one support services, please fill in a 1-1 HOT Support Referral Form and return to:
OR, if you have an NHS secure email address, you can email the form securely to firstname.lastname@example.org.
You can also return the form by post.
Unfortunately we cannot accept any referrals without this form.
Please note Health Opportunities Team offers young people a confidential service and we will not share information about what has been discussed with young people, unless the young person gives their agreement. However, we work within the National Guidance for Child Protection in Scotland (2014) and if we are very worried about a young person we may need to share some information with other people to keep them safe.
We hope that our support will help young people to:
- feel less distressed, less angry and more relaxed
- feel better about themselves
- have more control in their lives
- have better relationships with other people
- be more able to deal with stress
